Where do the categories come from?

The standard reasons include porosity, cracks, slag inclusion, lack of fusion, undercut, and misalignment. A project can add its own reasons.

Does this filter to a specific date range?

Yes. The period selector supports Q1 / Q2 / last 90 days as well as a custom range, and the dashboard recalculates accordingly.
